These forms are to be used only when the final (accepted) version of the paper is being transmitted to the Editor (i.e., after the manuscript has been reviewed & revised).

Copies of BOTH the Transmittal Form and Checklist PLUS the Consent to Publish/Copyright Release form must accompany the final accepted manuscript.
